This form is for withdrawing from the contract, not a generic complaints form. Use it to exercise the 14-day consumer withdrawal right.

If you buy as a consumer, you can withdraw within 14 days of the contract. SpotMyMap does not use a digital-download waiver for this product. Use the online withdrawal form or email support@spotmymap.com.
